Adjuvant chemotherapy versus adjuvant concurrent chemoradiotherapy after radical surgery for early-stage

摘要: We conducted a prospective study to assess the non-inferiority of adjuvant chemotherapy alone versus adjuvant concurrent chemoradiotherapy (CCRT) as an alternative strategy for patients with early-stage (FIGO 2009 stage IB–IIA) cervical cancer having risk factors after surgery. The condition was assessed in terms of prognosis, adverse effects, and quality of life. This randomized trial involved nine centers across China. Eligible patients were randomized to receive adjuvant chemotherapy or CCRT after surgery. The primary end-point was progression-free survival (PFS). From December 2012 to December 2014, 337 patients were subjected to randomization. Final analysis included 329 patients, including 165 in the adjuvant chemotherapy group and 164 in the adjuvant CCRT group. The median follow-up was 72.1 months. The three-year PFS rates were both 91.9%, and the five-year OS was 90.6% versus 90.0% in adjuvant chemotherapy and CCRT groups, respectively. No significant differences were observed in the PFS or OS between groups. The adjusted HR for PFS was 0.854 (95% confidence interval 0.415–1.757; P = 0.667) favoring adjuvant chemotherapy, excluding the predefined non-inferiority boundary of 1.9. The chemotherapy group showed a tendency toward good quality of life. In comparison with post-operative adjuvant CCRT, adjuvant chemotherapy treatment showed non-inferior efficacy in patients with early-stage cervical cancer having pathological risk factors. Adjuvant chemotherapy alone is a favorable alternative post-operative treatment.

Trimodality based on neoadjuvant chemoradiotherapy (nCRT) followed by surgery is gaining popularity as a treatment strategy for locally advanced esophageal cancer. In this review, we summarize the role of nCRT and the recommended nCRT regimens based on clinical trials and meta-analyses. We analyze the relationship of nCRT with pathologic complete response (pCR) and then identify potential predictive markers of response. Compared with surgery alone and neoadjuvant chemotherapy followed by surgery, trimodality provides longer survival and has the advantage of local control compared with definitive chemoradiotherapy. The standard regimen is a platinum-based regimen with a radiation dose range of 41.4–50.4βGy by conventional fractionation. Evidence shows that patients with pCR tend to live longer than non-responders, indicating that pCR is a significant prognostic factor for patients with esophageal cancer. Individualized medicine requires predictive markers of individual patients based on their own genes. Currently, no definite marker is proved to be sufficiently sensitive and specific for use in clinical practice, although 18-fluorodeoxyglucose positron emission tomography shows promise in predicting response to nCRT.

Concurrent optimization of structural topology and infill properties with a CBF-based level set method

摘要: In this paper, a parametric level-set-based topology optimization framework is proposed to concurrently optimize the structural topology at the macroscale and the effective infill properties at the micro/meso scale. The concurrent optimization is achieved by a computational framework combining a new parametric level set approach with mathematical programming. Within the proposed framework, both the structural boundary evolution and the effective infill property optimization can be driven by mathematical programming, which is more advantageous compared with the conventional partial differential equation-driven level set approach. Moreover, the proposed approach will be more efficient in handling nonlinear problems with multiple constraints. Instead of using radial basis functions (RBF), in this paper, we propose to construct a new type of cardinal basis functions (CBF) for the level set function parameterization. The proposed CBF parameterization ensures an explicit impose of the lower and upper bounds of the design variables. This overcomes the intrinsic disadvantage of the conventional RBF-based parametric level set method, where the lower and upper bounds of the design variables oftentimes have to be set by trial and error. A variational distance regularization method is utilized in this research to regularize the level set function to be a desired distance-regularized shape. With the distance information embedded in the level set model, the wrapping boundary layer and the interior infill region can be naturally defined. The isotropic infill achieved via the mesoscale topology optimization is conformally fit into the wrapping boundary layer using the shape-preserving conformal mapping method, which leads to a hierarchical physical structure with optimized overall topology and effective infill properties. The proposed method is expected to provide a timely solution to the increasing demand for multiscale and multifunctional structure design.

Concurrent fatigue crack growth simulation using extended finite element method

摘要: In this paper, a concurrent simulation framework for fatigue crack growth analysis is proposed using a novel small time scale model for fatigue mechanism analysis and the extended finite element method (X-FEM) for fatigue crack growth simulation. The proposed small time scale fatigue model does not require the cycle counting as those using the classical fatigue analysis methods and can be performed concurrently with structural/mechanical analysis. The X-FEM greatly facilitates crack growth simulation without remeshing requirements ahead of the crack tip as in the classical finite element method. The basic concept and theory of X-FEM was briefly introduced and numerical predictions of stress intensity factors are verified with reference solutions under both uniaxial and multiaxial loadings. The small time scale fatigue model is integrated into the numerical simulation algorithm for concurrent fatigue crack growth analysis. Model predictions are compared with available experimental observations for model validation.

摘要: 在分布式存储系统中,常用的数据冗余方法包括副本和纠删码(erasure code,EC)。相较于副本,EC具有更好的存储效率,但是在更新方面的开销更大。此外,并发更新带来的一致性和可靠性问题给EC应用带来了新的挑战。许多研究工作都致力于优化EC技术,包括算法优化、数据更新方法创新等,但并发更新的一致性和可靠性问题尚未得到很好解决。本文介绍了一种将数据更新与EC编码解耦的存储系统,命名为DDUC,并提出了一种副本与校验块结合的放置策略。对于(N, M)的EC系统,按照N组M+1的副本进行数据布局,并将同一条带的冗余数据块都放置在校验节点上,使得校验节点可以自主地执行本地EC编码。基于上述策略,实现了一种两阶段数据更新方法,在第一阶段按照副本模式进行数据更新,在第二阶段由校验节点独立完成EC编码。这样在保证高并发性能的同时,解决了并发更新导致的数据可靠性降低的问题。同时利用PMem硬件的字节寻址和8字节原子写特性实现了一种轻量级的日志机制,在提升性能的同时保证了数据的一致性。实验结果表明,和当前主流的存储系统Ceph相比,本文所提出的存储系统并发访问性能提升至1.70–3.73倍,时延仅为Ceph的3.4%–5.9%。
